Programs for Youth & Young Adults
Intentionally designed for youth (ages 12 - 26) without insurance and/or from low income communities, particularly those who identify as Black and Indigenous People of Color (BIPOC) and/or as part of the LGBTQAI+ community.
Youth Participants: “Choose Your Own Journey”
There is a relationship between how we tell our life story and how meaningful it feels. How we occupy our days, and how we define ourselves. C4CC participants are invited to choose your own journey with us.
Will you start with an OT-based exploration of your strengths and dreams, developing an individualized plan from there?
Will you ease into the Farm experience with horticulture and other farm activities?
Or will you engage in C4CC’s workshops (mindfulness, play therapy, music therapy, yoga, more) building relationships with C4CC staff, volunteers, and participants?
The choice is yours. You take the lead on decisions about your mental health and well-being - with the support and collaboration of C4CC. We help you determine what healing means to you rather than telling you how to heal.
Holistic, OT-Centered Mental Health Care
C4CC provides holistic, trauma-responsive care, starting with an occupational therapy-based exploration of youths’ strengths and challenges, followed by holistic and personalized mental health services.
Our Mental Health Provider Network of licensed professionals in therapy (occupational, music, recreation, play), counseling, social work, nursing, and wellness deliver innovative, evidence-based interventions tailored to each participant’s needs, coordinating with families and schools to ensure comprehensive support.
During C4CC’s pilot phase, services are provided (at no cost) to a limited number of youth ages 12 to 26 through our partnerships with Chatham County Schools, UNC Center for Excellence in Community Mental Health, Cross Disability Services (XDS), and the Farm at Penny Lane.
Therapeutic Workshops, Nature-Inspired Experiences, Special Projects
Integrating the healing power of nature, community, and connection into our therapeutic approach. Therapeutic workshops and farm-based activities aim to nurture the whole person and address mental, emotional, and social needs. In addition, we offer special projects and unique tools like:
Food2Mood Advocacy Project empowers young adults with any mental illness (e.g., PTSD, depression, anxiety, ADHD) to address food insecurity in our local community through advocacy, storytelling, and mental health support.
Mi Rialiti (“my reality”) is a tool C4CC participants will have access to tell their unique stories through audio, video, photo and text narratives plus 3D mapping, with the power to reauthor their own life narratives and impact community decision-making.
